1. Services Description

Renew Health provides preventive cardiovascular health services including carotid intima-media thickness (CIMT) imaging, advanced laboratory testing, metabolic assessment, and personalized cardiovascular prevention programs. Our services are provided by licensed physicians and healthcare professionals.

Our website provides general health information for educational purposes. This information does not constitute medical advice and is not a substitute for professional medical diagnosis, treatment, or care from a qualified healthcare provider.

2. Medical Disclaimer

The content on this website is for informational purposes only. Nothing on this site should be construed as medical advice, diagnosis, or treatment. 3. Patient Relationship

A physician-patient relationship is established only upon completion of a clinical intake process, review by a Renew Health physician, and mutual agreement to proceed with care. Submitting a contact form or requesting information does not create a physician-patient relationship.

Clinical services are provided pursuant to separate Patient Agreement and Consent forms, which govern the terms of your care. In the event of any conflict between these Terms and your Patient Agreement, the Patient Agreement controls with respect to clinical services.

4. Eligibility and Account Use

Our services are available to individuals 18 years of age and older. By using our website or services, you represent that you are at least 18 years old and have the legal capacity to enter into these Terms.

You agree to provide accurate, current, and complete information when communicating with us or scheduling services. You are responsible for maintaining the confidentiality of any account credentials.

5. Appointments and Cancellations

Scheduling an appointment through our website or by phone is subject to availability and confirmation by our team. We ask that you provide at least 24 hours notice if you need to cancel or reschedule an appointment. Late cancellations or no-shows may be subject to a cancellation fee as described in your Patient Agreement.

6. Fees and Payment

Fees for services are as described on our Memberships page and in your Patient Agreement. Payment is due at the time of service unless other arrangements have been made in advance. We accept major credit cards and other payment methods as specified at the time of service.

Renew Health does not currently bill insurance directly. We will provide documentation to assist you in seeking reimbursement from your insurer where applicable. We make no representation that our services will be covered by your health insurance plan.

7. Intellectual Property

All content on this website — including text, graphics, logos, images, and software — is the property of Renew Health, Inc. or its content suppliers and is protected by United States and international copyright, trademark, and other intellectual property laws.

You may view, download, and print content from our website solely for your personal, non-commercial use. You may not reproduce, distribute, modify, create derivative works from, publicly display, or exploit any content from this site without our prior written permission.
